Human error

The phrase ‘human error’ is often used in incident investigations to “explain” how the actions of one or more individuals caused the incident.

It is typically used when people made mistakes (often ones considered obvious or preventable in hindsight) or deviated from official procedure.

According to safety professor Sidney Dekker, there are two ways of looking at ‘human error’: The old view and the new view.
