This is a template I frequently use with teams to review the outcome of a project, with a focus on reflection of the process to identify strengths which should be maintained, and weaknesses to be addressed.
I typically have individuals contribute asynchronously to this ahead of time, followed by a meeting to discuss any differences of opinion and to commit to follow-up actions.

Did the scope change during the lifecycle of the project?
Does the end result match what we initially set out to do?
What went well?
What could’ve gone better?
Are there processes we should call out because they’re working well for us and should be sustained going forward?
Are there actions we can take to incorporate lessons from the project in our future work?
