Post-incident review
A post-incident review (PIR) process is the analysis of an incident, and evaluation of the incident management response, following a safety accident (broadly, Resilience Engineering) or systems outage (Site Reliability Engineering).
They are used to gain an understanding of what caused the failure, to learn from the incident and to document this for communication to stakeholders.
In certain fields, especially that of IT/SRE/DevOps the name postmortem is frequently used for this process. I’m not a fan of this: Perform a post-incident review instead of a postmortem
